INSTRUCTIONS: Filling out a PDF Application Form

    1. Download and install the latest version of Adobe Acrobat reader.Adobe Acrobat Reader
      (If unable to download this version, the form will also work with Adobe Reader 9.4.6. )
    2. Download and save a copy of the application PDF form to your computer to work from. Click “save as”, rename the file, and save the pdf form. CLOSE THIS SAVED APPLICATION FORM, THEN REOPEN.
    3. Double click on the PDF application form. This will start Acrobat Reader.

      *Apple Mac OS Users: Please ensure that Apple Preview software is not used to open the PDF application form. Only the latest version of Adobe Acrobat reader will work.

    4. Fill out the form by clicking in the fields. Use the tab key to navigate between fields.
    5. Do not copy and paste values into numeric fields. Type them in.
    6. Project Detail fields allow for rich text (bold, italicize, underline….). It is best to format text (11 pt minimum font size) in a Microsoft Word or another word processing software, then copy and paste formatted text into the PDF application form.
    7. Limit map image size to less than 1MB. Only .bmp, .jpg, .gif, .png, and .tiff files are accepted.
